Chernobyl nuclear accident
"The Chernobyl accident in 1986 was the result of a flawed reactor design that was operated with inadequately trained personnel and without proper regard for safety." See also: Three Mile Island.
Exxon Valdez oil spill
"The probable cause of the spill was the failure of the third mate to properly maneuver the vessel because of fatigue and excessive workload. Other contributing factors were one, the failure of the master to provide a proper navigational watch because of impairment from alcohol, two, the failure of the Exxon Shipping Company to provide sufficient crew for the Exxon Valdez, and three, the lack of an effective Vessel Traffic Service because of inadequate equipment and manning levels." Potters Bar rail crash
"A new report says poorly maintained points were the most likely cause of the Potters Bar rail crash. Seven people died and 76 were injured when the train derailed just outside the station on 10 May last year as it headed to King's Lynn, Norfolk."
